HIPAA NOTICE
We take participant privacy very seriously, and it is important to us that you are made fully aware of a potential privacy issue. Notice is provided by Volunteers of America, Dakotas that on June 26, 2024, it was notified of a breach of personal information of the following individuals by initials (full names where applicable) listed in this link. Two flash drives were turned in by a third party who reported they were with a laptop they recently purchased. Personal information on the flash drives was of name, address, Social Security number, etc. We continue to investigate this matter including by interviewing the former employee who owned the laptop with which the flash drives were found to determine if such person otherwise disseminated any of the PHI; by reviewing communications it receives related to you, and by notifying its employees of their obligations to maintain security for and confidentiality of PHI.
